The Problem With Pathology Reports
You open your pathology results and see rows of abbreviations: MCV, eGFR, GGT, HbA1c, ALT, TSH, MCH, MCHC, RDW. Each one has a number, a unit (mmol/L, fL, U/L, x10^9/L), and a reference range. Some have a small H or L flag. There is no explanation of what any of it means.
Your GP appointment is 15 minutes and covers other things too. "Everything looks basically fine" is a common summary — which may well be true, but it does not help you understand what your results actually say about your health.

Common causes: Fatty liver disease, regular alcohol, some medications (statins, paracetamol), recent strenuous exercise. Mildly elevated ALT is very common and often benign.
eGFR 54 mL/min/1.73m2 (L)
An estimate of how well your kidneys are filtering waste. A value of 54 is below the usual threshold of 60, but a single result is not enough to diagnose kidney disease.
Common causes: Can be transiently low after dehydration, intense exercise, or heavy meals. A persistent value below 60 on two tests, three months apart, would suggest reduced kidney function worth investigating.
HbA1c 47 mmol/mol (H)
Your average blood sugar over the past few months is in the prediabetes range. This is a warning sign but not yet a diabetes diagnosis.
Common causes: Prediabetes is strongly associated with excess body weight, physical inactivity, high-carbohydrate diet, and family history of T2 diabetes. Lifestyle changes at this stage can prevent progression.
TSH 6.8 mIU/L (H)
A raised TSH means your pituitary is working harder than normal to stimulate your thyroid. This pattern suggests your thyroid may be underactive.
Common causes: Hypothyroidism (underactive thyroid), autoimmune thyroiditis (Hashimoto disease), certain medications. Symptoms include fatigue, weight gain, cold intolerance, and constipation.
MCV 108 fL (H)
Your red blood cells are larger than the normal range. This is called macrocytosis. It is a clue, not a disease, and needs further investigation to find the cause.
Common causes: Vitamin B12 deficiency, folate deficiency, regular alcohol use, underactive thyroid, or certain medications. All four are treatable.
GGT 88 U/L (H)
A liver enzyme that is elevated above the normal range. GGT is the most sensitive marker for alcohol-related liver stress and bile duct problems.
Common causes: Regular alcohol consumption, fatty liver disease, bile duct obstruction, some medications (anticonvulsants, antidepressants). GGT can be elevated for weeks after a period of heavy drinking.
Why Blood Test Reports Are Written This Way
Australian pathology reports are formatted for clinical use, not patient use. A GP who reviews hundreds of results per week needs a dense, machine-readable format that can be scanned in seconds. Patient-friendly language, explanations, and context would make reports longer and harder to use in a clinical workflow.
Abbreviations
Clinical shorthand saves space on the report and matches what clinicians are trained to read quickly. ALT, eGFR, TSH, and MCV mean nothing to most patients but are instantly recognisable to a clinician.
SI units
Australia uses SI units (mmol/L, x10^9/L, fL, U/L) which are internationally standardised for clinical precision. They carry no intuitive meaning for most non-clinicians.
Reference ranges only
The report shows you whether your value falls within a population range. It does not explain what the range means, what causes values to be outside it, or what the clinical significance is.
No context for the reader

Why are blood test results so hard to understand?
Pathology reports are designed for clinicians, not patients. They use abbreviations (MCV, eGFR, GGT), SI units (mmol/L, x10^9/L, fL), and reference ranges with no context for what being outside them actually means. GP appointments are typically 15 minutes, leaving little time to walk through every result. The result is that most patients receive a document they cannot decode and a brief reassurance that everything is "basically fine."
What is a plain-English blood test explanation?
A plain-English explanation translates each marker from its clinical abbreviation into what it actually measures, whether your value is in a normal range for an Australian adult, what commonly causes values to be above or below normal, and what (if anything) typically follows from the finding. What does ALT mean on a blood test?
ALT stands for alanine aminotransferase, a liver enzyme. It leaks into the bloodstream when liver cells are stressed or damaged. The normal range in Australian labs is typically 5-35 U/L for women and 5-40 U/L for men. Mildly elevated ALT (up to 3 times the upper limit) is most commonly caused by fatty liver disease, alcohol, or certain medications. Significantly elevated ALT (more than 5-10 times normal) suggests more active liver injury and needs further investigation.
What does eGFR mean?
eGFR stands for estimated glomerular filtration rate. It estimates how well your kidneys are filtering waste from your blood, expressed as millilitres per minute per 1.73 m2 of body surface area. In Australia, an eGFR above 60 is generally considered normal in adults under 70. A single result below 60 does not confirm kidney disease; two results below 60, three months apart, are needed for a diagnosis of chronic kidney disease (CKD). eGFR naturally declines slightly with age.
What does HbA1c mean?
HbA1c (haemoglobin A1c or glycated haemoglobin) measures the average blood glucose level over the past 2-3 months. It reflects the proportion of haemoglobin molecules that have glucose attached. In Australia: below 42 mmol/mol (6%) is normal; 42-47 mmol/mol (6.0-6.4%) is prediabetes; 48 mmol/mol (6.5%) or above is diagnostic of diabetes. It is used both to diagnose diabetes and to monitor how well it is being managed in people already diagnosed.
