What Pushes an INR Up on Warfarin
If you take warfarin and your INR has moved above your usual target range, one of these is the likely reason. Antibiotics are by far the most common and the most dangerous.
Metronidazole, trimethoprim-sulfamethoxazole, ciprofloxacin
The most common trigger of a dangerous INR spike. These inhibit warfarin metabolism in the liver and kill the gut bacteria that make vitamin K at the same time.
Erythromycin, clarithromycin (macrolides)
Strongly inhibit the liver enzyme that clears warfarin. An INR recheck within a few days of starting is wise.
Amiodarone
A heart-rhythm medication that competes with warfarin clearance. The interaction can take weeks to fully develop and often needs a pre-emptive warfarin dose reduction.
Fluconazole and other azole antifungals
Potent inhibitors of warfarin metabolism, similar in scale to the interacting antibiotics above.
SSRIs (e.g. sertraline, fluoxetine)
Raise INR somewhat but also impair platelet function in their own right, compounding bleeding risk beyond the INR number itself.
NSAIDs and aspirin
Often will not move the INR much but add a separate stomach-bleeding and platelet risk on top of your anticoagulation — avoid unless your GP has specifically approved it.
St John’s wort
Included here because it is the reverse trap — it speeds up warfarin clearance and can drop your INR out of range, risking a clot. Fish oil, garlic, ginkgo and dong quai raise bleeding risk without necessarily moving the INR.
Cranberry juice and alcohol binges
Cranberry juice in large quantities can inhibit warfarin metabolism. A binge-drinking episode temporarily impairs the liver’s ability to clear warfarin.
Eating much less green leafy veg than usual
Warfarin works by blocking vitamin K. Less dietary vitamin K in a week means less to block, so the INR rises — this is why consistency matters more than avoidance.
Acute illness, fever, diarrhoea or poor appetite
Any acute illness can reduce vitamin K intake and absorption while also altering liver metabolism, making INR unstable until you recover.
New liver problems or thyroid overactivity
A newly unwell liver clears warfarin more slowly, and an overactive thyroid speeds up the breakdown of clotting factors — both push INR upward over days to weeks.
Causes of a High INR When You're NOT on Warfarin
This scenario is not the medication working — it is a genuine clue that something is interfering with your clotting factors and needs investigation.
Liver disease
The liver makes nearly all clotting factors, so a raised INR is one of the most sensitive markers of significant liver dysfunction. It is a component of both the MELD and Child-Pugh scoring systems used to grade liver disease severity.
Vitamin K deficiency
Seen with poor diet, malabsorption conditions such as coeliac disease or cholestasis, pancreatic insufficiency, or after prolonged courses of broad-spectrum antibiotics that wipe out vitamin K-producing gut bacteria.
Disseminated intravascular coagulation (DIC)
Seen in critically unwell hospital patients — sepsis, major trauma, some cancers. Usually accompanied by low platelets and low fibrinogen, and needs urgent hospital management.
Inherited factor VII deficiency
Factor VII sits specifically in the pathway INR measures, so a deficiency raises INR in isolation while other clotting tests stay normal. Usually picked up incidentally or after unexplained bleeding.
Rat-bait (superwarfarin) ingestion
Accidental or deliberate ingestion of long-acting rodenticides can cause a severe, prolonged INR rise that needs specialist reversal and monitoring for weeks.
Massive transfusion or fluid dilution
Seen after major surgery, trauma or large-volume resuscitation, when replaced blood volume contains proportionally fewer clotting factors than the patient started with.
Signs That Your INR May Be Too High
Many mild elevations cause no symptoms at all and are only picked up on a routine blood test. Watch for these signs between tests, especially in the days after a new medication, illness, or change in diet or alcohol intake.
Bruising more easily than usual
Small bumps that would not normally leave a mark start to cause visible bruises. Common and often the first sign that clotting has slowed.
Bleeding gums when brushing your teeth
A subtle but common early sign of a rising INR, especially if it is new and has not happened before on your usual dose.
Nosebleeds that take longer to stop
A nosebleed that will not stop after 20 minutes of firm pressure is a red flag and needs same-day medical attention.
Unusually heavy periods
A sudden increase in menstrual bleeding compared with your normal pattern can indicate your INR has climbed too high.
Blood in urine or stool
Pink or red urine, or black, tarry, foul-smelling stools, both indicate internal bleeding and need urgent same-day review.
Vomiting blood or coffee-ground material
A sign of bleeding higher in the digestive tract. This is a medical emergency — seek immediate care.
Large, unexplained bruises with no injury
Bruising that appears spontaneously, is unusually large, or clusters in one area without a matching injury suggests the INR is significantly out of range.
Sudden severe headache, confusion or weakness on one side
These can be signs of bleeding inside the skull. Call 000 or go straight to an emergency department — do not wait to see if it settles.
Red Flags — When a High INR Is an Emergency
This is a bleeding-risk marker, so it is worth being blunt: the situations below are not “mention it at your next appointment” situations. They need same-day attention or an emergency department, as noted.
INR above 5
Call your GP or anticoagulation clinic the same day, even if you have no bleeding and feel completely well. The risk of spontaneous bleeding rises sharply above this level.
INR above 9
Needs urgent same-day medical review, almost always with vitamin K given to bring the level down safely and quickly.
Any significant bleeding, at any INR
Blood in urine, black tarry or bloody stools, vomiting blood or coffee-ground material, coughing up blood, a nosebleed that will not stop after 20 minutes of pressure, unusually heavy periods, or bleeding gums.
A head injury or fall while on warfarin — even a minor one
Call 000 or go straight to an emergency department, even with no symptoms at all. A bleed inside the skull can develop hours later, which is why warfarin head injuries are always treated seriously.
Sudden severe headache, confusion, one-sided weakness, or a change in vision
These can indicate bleeding inside the skull and are a medical emergency — call 000 immediately.
Large unexplained bruises, or severe abdominal or flank pain
Bruising with no matching injury, or new severe abdominal or flank pain, can both indicate internal bleeding and need same-day assessment.

The pathway splits early depending on whether you take warfarin. Steps 2-4 apply if you are on warfarin; steps 5-8 apply if you are not and the cause is still unclear.
Confirm the result and check for bleeding
Your GP or anticoagulation clinic will first confirm there is no active bleeding and ask about recent illness, missed or doubled doses, new medications, alcohol, and diet changes — most explanations turn up in this conversation alone.
For a mildly high INR on warfarin: hold a dose and recheck
One or two doses are typically withheld and the INR is rechecked within a few days, rather than making an immediate large dose change based on a single result.
For a higher INR on warfarin: oral vitamin K
A small oral dose of vitamin K brings the INR down over 24-48 hours without overshooting into the clot-risk range that comes from over-correcting.
Serious bleeding: hospital reversal

Not on warfarin: repeat PT/INR with APTT and fibrinogen
A repeat panel confirms the result was not a one-off lab artefact and checks whether the intrinsic pathway (APTT) and fibrinogen are also affected, which narrows down the likely cause.
Not on warfarin: liver function tests and albumin
Since the liver makes almost every clotting factor, LFTs and albumin are checked early to look for underlying liver disease as the cause.
Not on warfarin: full blood count, platelets and a vitamin K trial
An FBC and platelet count help exclude DIC, while a trial of vitamin K checks whether deficiency was the cause — the INR should improve within a day or two if it was.
Haematology referral if it stays unexplained
If liver tests, vitamin K trial and infection screening are all normal and the INR remains raised, referral to a haematologist is warranted to look for an inherited factor deficiency or another bleeding disorder.
Staying Stable on Warfarin — Practical Tips
Keep green vegetable intake consistent, not avoided
Consistency beats avoidance. Eating the same rough amount of leafy greens week to week keeps your vitamin K intake — and therefore your INR — stable. Suddenly cutting them out is what causes INR to swing.
Tell every prescriber and pharmacist you take warfarin
Do this before starting any new medicine, including over-the-counter painkillers and herbal or complementary products — many of the biggest INR shifts come from a new prescription nobody checked against warfarin.
Keep alcohol steady and moderate
A sudden binge, or suddenly stopping regular drinking, both destabilise warfarin metabolism. Steady, moderate intake in line with NHMRC guidelines is easier for your dose to account for.
Never double a missed dose
If you miss a dose, take the next one as scheduled rather than doubling up — doubling risks pushing your INR sharply upward.
Never adjust your own dose based on a number you read online

They can still shift an INR result unpredictably if it is measured for an unrelated reason, which is why doctors do not use INR to judge whether these medicines are working properly. Should I ever adjust my own warfarin dose based on my INR?
No. Never stop, skip, double or otherwise change a warfarin dose on your own because of a number you have read on a pathology report or online. An INR that is too low carries its own serious risk — stroke or blood clot — which can be more dangerous than the bleeding risk from a high one. Dose changes should only be made by your GP or anticoagulation clinic, who will look at the trend, your diet, other medications, and any recent illness before deciding whether to hold a dose, adjust it, or give vitamin K.
